Thank you for your interest in Formulary 55!  

To start the approval process, you will need to create an account by going to the Wholesale Login Page  and follow the link to " Sign up here. ". Complete the form and tell us about your shop, other lines you currently sell, links to your shop's website and/or social media presence. If your store's physical location is different than the shipping address provided in the application, please provide that as well.

You must have a brick & mortar storefront to sell Formulary 55 products. If you would like to sell our products both in your store and on your website you must contact Formulary 55 and become approved before selling online. We also partner with select gift box companies where Formulary 55 products are included as part of a curated gift box. 

We have also partnered with Faire, a wholesale marketplace, and you can place your orders on our entire line once you establish an account.
